Our goals:

Who we are:

NCHP is a non-profit organization that has been home to a variety of federal, state and local programs and services over the 30 years it's been in existence. We have a variety of programs including: Veterans on the brink of homelessness with our Housing for Heroes program, a COVID relief program funded through HRSA, and we just announced our partnership with the Department of Veteran Affairs to bring action and awareness to Veteran Suicide Prevention through our new program: The Best is Yet to Come, funded through the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program. Based in Tucson, Arizona, NCHP—formerly Border Health Foundation—was founded in 1984 to address the health needs of border populations. Since then we have developed a strong national presence providing education and services to minority and underserved populations in 47 states and two U.S. territories.

We have housed over 82 public health programs and services promoting the health and well-being of individuals and communities across the U.S. It is our goal to improve health outcomes through innovative services and compelling educational programs. NCHP ESGR Patriot Award

National Community Health Partners (NCHP) was recognized by the Arizona Office For Employer Support of The Guard And Reserve, with the ESGR Patriot Award. This acknowledgement affirms NCHP support for the brave men and women and their spouses that served our great Nation. NCHP further affirmed our support with the Statement of Support for the Guard and Reserve.

NCHP's National Community Housing SSVF program is CARF accredited! CARF International accreditation demonstrates a program’s quality, transparency, and commitment to the satisfaction of the persons served.
